MASTERMIND INNER CIRCLE AGREEMENT OF TERMS AND CONDITIONS

By enrolling in the Home Care Breakthrough Inner Circle Mastermind Group, you acknowledge the following:

I am purchasing enrollment into The Grace Project llc dba Home Care Breakthrough Solutions training services. I am comfortable with the things I’ve learned about this program and the conversations I’ve had with the Home Care Breakthrough team. No performance claims have been made. I am entering into this agreement without any duress from any representative at Home Care Breakthrough Solutions.

I acknowledge that I am of sound mind and body and attest that I act on behalf of myself; meaning no agent or attorney-in-fact has Power/Letter of Attorney over my personal affairs.

I acknowledge that my level of success is dependent upon a variety of factors, including such things as my level of commitment to the program, my ability to implement what I learn, and my own initiative.

I acknowledge that this is a 12 month program and that I am financially responsible for my commitment to participating in this group. If for any reason I miss a payment, or my card expires or gets declined, I will rectify my missed payment immediately. Home care breakthrough DOES NOT do automatic renewals.

Education Services:

In exchange for any monetary considerations that I’ve approved as part of my participation in this program, my purchase includes:

● 1-1 Personal strategy kick-off call with Gregg

● Access to the Home Care Breakthrough Mastermind vault and all of the trainings inside of the vault. I acknowledge that I only have access to the materials while I am part of this program. I also recognize that these materials are copyrighted and are not to be shared with anyone outside of my immediate organization. I also agree to not duplicate any of the materials without express written consent from Home Care Breakthrough Solutions

● Live group Q & A with Gregg weekly (or occasionally with another team member). Q & A sessions will always be held except during rare circumstances (This will include a minimum of 45 sessions throughout the year)

● Live Mindset Monday calls with Mandy, Gregg, or another team member. These sessions will be held weekly except on rare circumstances (This will include a minimum of 45 sessions throughout the year). These sessions are approximately 20-30 minutes

● 2x live events with other Mastermind members (dates will vary and one of those events is with Mastermind members only

● Private group access for questions through VOXER during the week with other Mastermind members

● Access to Gregg’s documents, SOPS, or network as he deems fit

● Access to advanced sales group training every other week, except rare occasions) –minimum of 22 sessions over the course of the year

● Access to group recruiting training about every other week, except rare occasions)- minimum of 22 sessions over the course of the year

● A minimum of 5 Virtual half day Mastermind sessions with just your Mastermind group

● Attend any of Gregg’s virtual trainings during the year for free

This is a non-refundable program. Any refunds or rescission is pursuant to applicable law or as set forth within our Satisfaction Warranty after full completion of the 12-month program.

Satisfaction Warranty:

You are potentially eligible for this warranty of service if you do not generate at least the amount you invested in this program within the next 12 months.

If that were to occur, You may request a pro-rated refund.

This pro-rated refund would be the purchase price of your training (and/or coaching) minus any revenue you generated as a result of your efforts and implementation of the strategies suggested, not including any associated business costs, advertising costs, or any other expenses incurred.

In order to be eligible for this warranty, you must commit to fully participating in the program. To show the required level of participation, you (or someone on your team) must attend:

At least 10% of the Mindset Mondays sessions
At least 25% of the group Q & A sessions To ensure All of your questions get answered
At least 15% of the advanced sales and/or recruiting group sessions (you or your team)

(we will also accept you having watched the replays – which we can track inside of the Mastermind membership site)

You must have shown a diligent effort to implement the initiatives of your strategic plan that is discussed on your strategy call with Gregg; This includes watching any training videos that were suggested, and then showing that you have implemented the actual strategies suggested, made the sales calls, and/or implemented the recruiting solutions. We may ask for documentation showing this was completed.
You must complete your Mastermind weekly check in report for at least 80% of the weeks you participate in the program (this takes about 5-7 minutes each week)
Additionally, you must contact us immediately if you are not seeing results, are unhappy, or are dissatisfied at any point during the process so we can help optimize the program for better results. You can message me at [email protected]

Failure to participate in the above guidelines and/or submit the necessary documentation, and/or failure to attend the % of calls outlined above, will eliminate your eligibility for this warranty without exceptions.

If, after completing the above items for 12 consecutive months, you do not feel our services have added value to help generate enough revenue to at a MINIMUM match your investment, we will refund you the difference.

To be eligible you must provide documentation of your revenue results (i.e., bank statements, payment processing statements, and/or P & L statements, etc.).

Engagement Period:

I understand and agree that, by checking the box saying that I’ve read and agree to the “terms and condition” , I will have my credit card(s) charged (or check cashed) for the participation rates described in this agreement as based on my selected investment option for my inclusion in these programs. I authorize The Grace Project dba Home Care Breakthrough Solutions to charge my card(s) as indicated in this enrollment farm. It is my responsibility to make sure I meet my deadlines and keep my scheduled appointments. I understand that my participation in the Program and the Sessions expire at the end of the twelve (12) month term (the ”Engagement Period"), beginning from the date listed on Page 1 or 2 of this enrollment form. This is a 12-month non-cancellabIe commitment. Any unused sessions and/or bonuses/benefits are lost and will not carry-over beyond the 12-month Engagement Period unless approved in writing by Gregg Mazza. I understand and agree that if I miss, fail to attend or do not make use of any scheduled coaching/consulting days, coaching/mastermind sessions, mastermind gatherings (collectively the “Sessions" ), coaching calls, emergency calls, online training courses, and/or any/all associated bonuses/benefits, that I forfeit any/all parts of the Program (the “Program" ) as Sessions will not be rescheduled, nor will I be given the opportunity to attend make-up Sessions unless agreed to in advance. I understand and agree that by completing and signing this Agreement that I am committing to the full Engagement Period of the twelve (12) month period, as of the date of my acceptance in the Program. I further understand and agree that this is a twelve (12) month non-cancellable commitment and if, for any reason, I choose to remove or cancel myself out of the Program prior to the end of the twelve (12] month Engagement Period, I am nonetheless obligated to pay any outstanding balances, including any/all monthly payments, due under this Agreement as listed on Page 1 and/or will not receive any refund (partial or full) for any reason whatsoever for any monies paid or credit card charges. Participant understands that there will be no delaying, Stalling or freezing of any Program participation and/or payments (partial or full) for any reason whatsoever including personal, professional and/or financial reasons. Participant warrants he/she will not affect any credit card chargebacks regarding this purchase. Participant warrants he/she will not affect any cancellation of credit card(s) to avoid obligated payments regarding this purchase (partial or full) for any reason. Further, I (participant) understand that I am solely and completely responsible for any indebtedness incurred as a result of my entering into this agreement. Whether I choose to assign full or partial rights (i.e. deciding to partner with another individual) hereunder, or delegate my obligations under this Agreement, I understand that I am responsible for the complete and total balance amount due to the Company.

Intellectual Property/Privacy Rights:

I understand that the Company has sole discretion to terminate agreement and remove me from continuing in the program at any time for any reason without a refund (partial or full) if I am disruptive (as defined by the Company), if I infringe on any intellectual property and/or privacy rights of the Company, its members, participants, presenters and/or vendors/sponsors, if I am (or become) difficult to work with (as determined by the Company), if I cease to follow any of the Program guidelines, or if I in any way violate any rules set forth by the Company.
